One of the biggest deterrents to efficiency is holding on to systems that no longer fit. It can be as simple as what you regularly carry in your purse or briefcase, on up to a corporate-wide policy. And you may not even realize you have a system in place until a change occurs that throws it out of whack.
That occurred for me when I bought an iPhone. I was immediately in love with my new phone. Not only was it easy to use, but suddenly I had access to my email when I was out of the office. In fact, it was easier to check email than my voice mail. Which was fine, except I had told all my clients that the best way to reach me quickly was to call my office number, which I regularly checked for messages. Time to update what I tell my clients - I have a new system.
